You may have recently received a consent form from your Group Home or Adult Day Training (ADT) Provider asking you to authorize the release of information to Orion Quality Consulting. We understand that consent forms can sometimes feel confusing or overwhelming. Our goal is to make the process as clear and comfortable as possible. The information you choose to share will be handled responsibly, respectfully, and in accordance with applicable privacy laws and regulations. This page explains why the consent form is needed, what you should review before signing, what happens after the form is completed, and who to contact if you have questions, would like to make a change, or decide to withdraw your authorization.
Start with the relationship
The Waiver Provider receives the consent form and is responsible for giving it to the individual or Guardian. The Provider is also the first point of contact for questions about why the form is being requested, which records are needed, and how a decision may affect the Provider's process.
The Provider identifies the individual, reviews the form, obtains the appropriate signature, and confirms that the consent is complete.
A completed consent allows the Provider to share the selected information with Orion Quality Consulting for the purpose described in the form.
Orion Quality Consulting uses authorized information to perform consulting, reporting, planning, quality assurance, and related work assigned by the Provider.
What the form does
The consent allows Orion Quality Consulting to receive and use information selected on the form for the Provider work described in the consent. Information may be shared electronically, through paper records when necessary, or through conversations with the Provider.
The purpose is to allow Orion Quality Consulting to organize and analyze relevant information, prepare Provider work products, identify patterns or gaps, and support the Provider's own quality systems.
Information on the consent
The consent uses checkboxes so the requested categories can be identified. Not every category is needed for every assignment. The Support Plan is generally required because it establishes the individual's goals, services, preferences, and support needs for Provider planning and reporting.

The requested information should relate to the work the Provider assigned. You may ask the Provider why a category is selected and whether the requested scope can be clarified or limited.
After the form is completed
Ongoing consent
Consent is managed on an annual cycle. Automated reminders support the renewal process, but the Provider remains responsible for obtaining and maintaining a current consent.
Review the new form each year rather than assuming the categories, dates, or purpose are unchanged.
Contact the Provider if the individual's information, selected categories, dates, or records appear inaccurate or incomplete.
Send consent changes or revocation requests to consents@orionqualityconsulting.com and notify the Provider.
